What is the PREVENT score?

PREVENT stands for "Predicting Risk of cardiovascular disease EVENTs." It is a set of risk equations developed by the American Heart Association to estimate a person's chance of developing cardiovascular disease over the coming years. Introduced in 2023 and formally published in Circulation in 2024, PREVENT is the modern successor to the 2013 Pooled Cohort Equations — the tool many clinicians still call the "ASCVD risk score."

The 2026 ACC/AHA dyslipidemia guideline adopts PREVENT as the preferred tool for estimating cardiovascular risk. In practice, that means the number your doctor uses to decide whether to start a statin, intensify blood-pressure treatment, or simply keep monitoring is increasingly coming from PREVENT rather than the older equations.

Why PREVENT replaces the old Pooled Cohort Equations

The 2013 Pooled Cohort Equations served cardiology for a decade, but they had well-documented limitations. They were derived from roughly 25,000 people in older cohorts, they used race as a mathematical input, and they tended to overestimate risk for many contemporary patients. PREVENT was built to fix each of these problems and was derived and validated on a far larger, more modern population of about 3.3 million US adults.

Removing race was a deliberate scientific decision. Race is a social rather than a biological category, and hard-coding it into a risk formula could systematically distort estimates. PREVENT instead relies on measurable biology — including kidney function — to capture the same risk signal more fairly.

What PREVENT measures

Rather than a single number, PREVENT produces a richer picture. It estimates your risk over two time horizons — the next 10 years and the next 30 years — which is especially useful for younger adults whose 10-year risk looks low but whose lifetime trajectory may not be.

It also separates the type of disease. PREVENT reports total cardiovascular disease and breaks it into ASCVD (atherosclerotic disease — chiefly heart attack and stroke) and heart failure. Estimating heart failure risk is genuinely new: the old Pooled Cohort Equations did not do it at all.

The inputs it uses

  • Age and sex
  • Total cholesterol and HDL cholesterol
  • Systolic blood pressure, and whether you take blood-pressure medication
  • Whether you currently take a statin
  • Diabetes
  • Smoking status
  • eGFR (estimated glomerular filtration rate — a measure of kidney function)
  • Body mass index (BMI)

How the risk categories are read

Once PREVENT produces a 10-year risk percentage, the ACC/AHA framework sorts it into four bands that help guide the conversation about treatment:

  • Low risk: under 5% — lifestyle focus, generally no drug therapy needed.
  • Borderline risk: 5% to 7.5% — consider risk-enhancing factors before deciding.
  • Intermediate risk: 7.5% to 20% — statin therapy is often discussed.
  • High risk: 20% or above — intensive risk-factor treatment is usually recommended.

Because PREVENT tends to produce lower and better-calibrated estimates than the old equations, some people who were previously labelled "intermediate risk" may now fall into a lower band. That is not a loosening of standards — it reflects more accurate math applied to a contemporary population.

Using your result wisely

A PREVENT score is a starting point for a conversation, not a verdict. Two people with the same percentage can warrant very different plans depending on family history, coronary calcium scores, inflammatory markers, and personal preferences. Guidelines call these "risk-enhancing factors," and they matter most in the borderline and intermediate bands.
